60 minutes face to face assessment + 15 minutes clinical note taking and follow up.

(All clients are provided with an initial assessment report to provide a baseline and guide for ongoing   services).

Clinic Therapy sessions

(45 minutes intervention + 15 min   clinical notetaking and follow-up)

(30 minutes + 15 min clinical notetaking   + travel)

Subsequent reports, letters, NDIS progress reports, progress reports, form completion, home programs and functional capacity reports.

Assessment Administration Fee

(A fee applies to selected questionnaires for administration and scoring) e.g., Vinelands, Sensory Profile, Movement ABC etc. Charged in 15-minute intervals.

Phone calls to stakeholders.

e.g., school, support workers, NDIS case coordinators, and other Allied Health staff. (charged in 15-minute   intervals)

Goal Setting Case Conference

(It is recommended that   parents/carers meet with the OT at least 12 monthly without the child to   discuss progress and review their child’s goals). 45 minutes face to face +   15 minutes clinical paperwork.

 We understand that unplanned issues can arise, and you may need to cancel or reschedule an appointment. Should this occur, we respectfully ask that parents/carers contact Piccolo Kids via SMS 0491 247 747 or via email reception@piccolokids.com.au. The cancellation notice period is based on business days when notification is received, therefore excludes weekends and Mondays.

All cancellations are required to notify Piccolo Kids 48 hours before their appointment to avoid being charged. In the instance that the outlined conditions are not met, the client will be charged the full-service fee. No-shows will automatically be charged the full-service fee.

If an unexpected illness arises, please do not attend your appointment in person. We will be able to provide alternative therapy options to ensure the safety of all staff, practitioners, and clients.

The Occupational Therapist wants to be available for your needs and for the needs of all their clients. When a client does not show up for a scheduled appointment, other clients lose the opportunity to be seen. We appreciate your understanding.

It is crucial that you notify Piccolo Kids of your cancellation, at least 48 hours before your         appointment time to avoid being charged the full-service fee.
